Misato City conducts health examinations to prevent illness related to life style and cancer checkups for early detection. For more information, please consult the Health Guide called Kenko no Shiori which is published every March or Misato City official web site.

Mother and Child Health Handbook 母子健康手帳

Submit a Pregnancy Notification Form when you are pregnant. You will receive the Mother and Child Health Handbook and subsidy coupons for prenatal checkups. You need to state the expected birth date, the weeks of pregnancy, a medical institution’s name and address etc. on the Notification. Pregnancy Notification form is available at a section in charge.

<Requirements for application>

In accordance with the revised law, you are required to enter your Individual Number (My Number) and present your identification when you submit the Pregnancy Notification on and after January 1, 2016.

 *Required documents when a mother-to-be reports the pregnancy for herself

 Individual Number Card or Individual Number Notification Card and her ID such as 
 Residence Card, driver’s license or passport

*Required documents when a proxy of a mother-to-be reports the pregnancy

- ID of the proxy such as Residence Card, driver’s license or passport

- The mother-to-be’s Individual Number Card or Individual Number Notification Card, ID such as Residence Card, driver’s license or passport and a letter of proxy (a form is available for download on Misato City official website.)

Health Checkup for Children 子どもの健康診査

Misato City conducts health checkups for children. You should have your children take a health checkup at the designated time. You will receive a letter with questionnaires in late previous month of the checkup:

For 4-month-old infants

Pediatric practice, physical measurement, health interview and developmental check, lecture about how to wean, individual nutritional counseling (applicants only) and child-rearing consultation *book-start (book reading and gift book)

For 9-month-old infants

Physical examination, physical measurement, health interview and developmental check, child-rearing consultation and nutritional counseling (applicants only)

For 1-year-8-month-old infants

Physical examination, dental checkup, physical measurement, health interview and developmental check, child-rearing consultation, tooth brushing instruction (Please bring a toothbrush, a cup and a hand towel.) and nutritional counseling (applicants only)

For 3-year-6-month-old infants

Physical examination, dental checkup, visual checkup (if necessary), urine test, physical measurement, health interview, developmental check, child-rearing consultation, tooth brushing instruction (Please bring a toothbrush, a cup and a hand towel.), fluoride varnish (applicants only) and nutritional counseling (applicants only)

Immunization for Children 子どもの予防接種

Immunization is one of the effective ways to fight infectious diseases by administering a vaccine which helps our body to develop immunity.

Children living in Misato City can receive each vaccination individually at a designated medical institution when they are in good health.

For those who have a targeted child, Misato City provides a Japanese version of the “questionnaire for vaccination” which is interview sheets to fill in regarding your child’s health condition etc. on the day of immunization and a Japanese version guidebook, “Vaccination and Children’s Health” which describes in detail immunizations by post as stated schedule below.

Please read it carefully and thoughtfully to understand the effects and side effects before you take your child for vaccination.

Those who were born in Misato City:

Distributed at the notification of the birth at the counter of Misato City Hall or sent by mail about 1 month after the child’s birth.

Those who moved into Misato City:

After Moving-in procedure, distributed at the counter of Health Promotion Division or sent by mail about 1 month after the date of moving-in. (Contact us when you wish your child to have received vaccination soon after moving-in.)

Fee: For required immunizations designated by law, Misato City will cover the expenses for vaccinations administered within the designated immunization periods.

What to bring: Mother and Child Health Handbook, the questionnaire sheets for the vaccination and your health insurance certificate (which shows your child’s address and age).

As some institutions may administer by appointment, please confirm availability by phone etc. in advance.

If you cannot have your children immunized at the designated institutions due to a particular reason such as childbirth in your home country or illness, please contact the Health Promotion Division.

Note: Work out a plan with your family doctor about the schedule of vaccination. When you wish to have more information about vaccination, feel free to inquire by phone or contact us at your child’s health checkup.

Influenza Vaccination For Senior Citizens
高齢者のインフルエンザ予防接種

During the period of vaccination, apply for the influenza vaccination at a designated medical institution individually. The individual notice is not given. This vaccination screening questionnaire is available at the designated medical institutions in Misato City. Please consult the September issue of the official newsletter, Koho Misato.

Eligibility

    1) Citizen aged 65 or over

2) Citizen aged between 60 and 64, who is extremely restricted from doing his/her personal daily life activities because of disability in the cardinal, kidney or respiratory functions or who is disabled to lead a normal life due to the immune dysfunction resulted from a human immunodeficiency virus (which is equivalent to the certified first degree of physical disability). Consult at the Health Promotion Division or your family doctor for details.

Where to administer

Designated medical institutions. *Be sure to inform the Health Promotion Division to take a procedure prior to your visit to a medical institution other than the designated one.

Period of vaccination:

  From Saturday, October 15 to Sunday, December 25, 2016 (for FY2016)

Fee: With co-payment

(Exempted if you are a member of a household receiving Public Assistances or if you are a recipient of benefit for Japanese who had been left behind in China.)

 

Number of vaccination:

Maximum of 1 dose for a season. All fees will be on your own account, if you wish to receive an additional dose.

What to bring:

A document which shows your address and date of birth (e.g. Health Insurance Card or Driving License)

  *Official certificate or documents is needed if you are:

     - one who is 2), bring the physical disability certificate.

- a member of a household receiving a recipient of public assistance or a recipient of benefit for Japanese who had been left behind in Chain, bring the beneficially certificate.

How to receive the vaccination:

[At the designated medical institutions]

      1) Call and make a reservation at the institution.

2) Bring the certificate to prove your current address, your date of birth and other
    certificates (if necessary) and visit the institution.

[At the medical institutions outside of Misato City]

1) Please visit the Health Promotion Division with the name of the institution you would like to receive the vaccination, its address and doctor’s name. If you cannot come to the Health Promotion Division, the telephone inquiry is available.

2) Please bring the vaccination coupons issued by Misato City and the certificate to prove your current address, your date of birth and other certificates (if necessary) and visit the institution.

Note: When you receive the vaccination outside of Misato City, please contact Health Promotion Division in advance. Without taking proper procedure, you need to bear the full expenses.

Pneumococcal Vaccination for Senior Citizens
高齢者の肺炎球菌ワクチン予防接種

Pneumococcal disease is caused by a bacterium known as Streptococcus pneumoniae or pneumococcus. Many people have the bacteria in their nose or throat at one time or another without being ill. Pneumococcal bacteria spread from person-to-person by direct contact with respiratory secretions, like saliva. Pneumococcus can cause serious complications such as bronchitis, pneumonia or septicemia.

Applicable Citizen:

Note: You are NOT eligible for the vaccination as mandated one, if you have already received pneumococcal vaccination called ‘Pneumovax NP (23-valent pneumococcal capsular polysaccharide vaccine)’.

1) One who turns under mentioned age in FY 2016 (as of March 31, 2017).

       Age 65: Born from April 2, 1951 to April 1, 1952

       Age 70: Born from April 2, 1946 to April 1, 1947

       Age 75: Born from April 2, 1941 to April 1, 1942

       Age 80: Born from April 2, 1936 to April 1, 1937

       Age 85: Born from April 2, 1931 to April 1, 1932

       Age 90: Born from April 2, 1926 to April 1, 1927

       Age 95: Born from April 2, 1921 to April 1, 1922

       Age 100: Born from April 2, 1916 to April 1, 1917

Misato City subsidizes the optional immunization fee
三郷市任意予防接種費用の一部助成

Misato City subsidizes a portion of the optional immunization fee designated by the City.

[Eligibility and times of immunization]

Eligibility: Misato City resident on the day of immunization and aged mentioned in the table below.

Period: Vaccination is from April 1, 2016 to March 31, 2017

[Optional Immunization]

The Optional Immunization is an approved vaccination, but it is not Immunization designated by law. Be sure to think carefully and understand the expected effects, its side effects and Relief System for Injury to Health with Vaccination before you have your child take a vaccination.

Subsidy for Donor of Bone Marrow Transplantation
骨髄移植ドナー助成金交付

Misato City subsidizes those who donate bone marrow and peripheral blood stem cells (hereinafter ‘bone marrow’) so that the City promotes donor registration and transplantation of bone marrow.

Eligibility

1) One who has finished bone-marrow-donation under a bone-marrow-donation project promoted by Japan Marrow Donor Program (hereinafter JMDP) and received a certificate about bone-marrow-donation issued by JMDP on and after April 1, 2014.

     2) One who was a registered resident in Misato City on the day of bone-marrow-donation.

3) One whose employer (company or organization) offers no paid ‘donor-leave’ and has not received any other subsidy related to the bone-marrow-donation.

Subsidy

Subsidy will be the sum of the number of the days you visit the hospital and the number of hospitalized days multiplied by JPY 20,000 (per day) for the purpose of your bone-marrow-donation but not exceeding the total amount of JPY 140,000 per one bone-marrow-donation.

2) Hospital visit for autologous blood collection

3) Hospitalization for bone marrow harvest
4) Hospital visit, hospitalization or interview related to your bone-marrow-donation recognized by JMDP as necessary.

*Hospital visits for treatment of health damage caused by the bone marrow harvest or its related medical measures are excluded.

Application period for subsidy

The application period for subsidy is 90 days after the day when you finish bone-marrow-donation. If you have difficulty to apply within the limited time for any compelling reason, please consult us.
