This team provides intensive support for people with dementia or suspected dementia living at home who are 40 years of age or older in principle. Specialists coordinate hospital visits, support the use of long-term care services, provide advices to family members and connect them to medical and long-term cares.

This center provides information on house call doctors and care managers to those who wish to receive home medical care, as well as consultation about diseases and nursing care.

Watch over through meal delivery/ 見守り配食サービス
This service delivers lunch and dinner to elderly persons living alone who need to be watched over regularly and have difficulties securing meals on a daily basis and checks their safety at the same time (part of the cost to be borne by the user).
Emergency call system/ 緊急通報システム
Emergency call device is available for lending to elderly persons living alone. In the event of a sudden illness or other emergency, a call for help can be made 24 hours a day just by pushing the emergency button (part of the cost to be borne by the user).
Welfare telephone for the elderly/ 老人福祉電話
A telephone instrument is available for lending to the elderly persons living alone needing a telephone for safety checks, emergency calls, etc. (with income restrictions).
Home-visit hairdressing service/ 訪問理美容サービス
A barber visits the home twice a year to provide hairdressing and hair cutting services to those aged 65 years or older having difficulty going to a barber or beauty salon and who are categorized in the level 4 or 5 of the necessity of long-term care classification under the Long-Term Care Insurance Act.
Disposable diaper provision/ 紙おむつ支給
Disposable diapers will be delivered monthly (with a partial co-payment) to those aged 65 or over who need diapers because of constant incontinence who are classified as the level 3 to 5 in the necessity of long-term care classification, or who are classified as requiring nursing care or higher level under the Long-Term Care Insurance Act and certified as requiring full assistance for urination and defecation, and who are not receiving Public Assistance stipulated under the Public Assistance Act (part of the cost to be borne by the user).
Allowance for families in long-term care/ 家族介護慰労金支給
An allowance will be paid to a family member who cares at home a person aged 65 or over with a certification for the necessity of long-term care classification level 4 or 5 stipulated under the Long-term Care Act continuously for one year or longer, who has not used long-term care services for the past year, and has not been hospitalized for 3 months or longer.
Subsidy for Use of Detective Device for Wandering Elderly etc./ 徘徊高齢者等探索システム利用補助
The fee for search system will be subsidized for families using the device to help finding their wandering elderly.
Distribution of emergency medical information kits/ 救急医療情報キットの配布
Emergency medical information kits will be distributed to the elderly aged 65 or over etc. living alone to store information on their family doctors and daily medications in their refrigerators in the event of a sudden illness or other emergency.
Light level of living support program/ 軽度生活援助事業
This program dispatches home caregivers to frail elderly persons aged 65 or over to provide light assistance in daily living (with user’s co-payment).
Short accommodation for a life management guidance/ 生活管理指導短期宿泊
Those elderly having anxieties about their daily lives and/or interpersonal relationships with neighbors can stay temporarily in a care-house in Misato City to improve their life style habits (with user’s co-payment).
General programs for long-term care prevention/ 一般介護予防事業
This program provides classes and places for the elderly to go for exercise, oral care, dementia prevention, etc. to prevent them from becoming in need of assistance or care and to maintain a healthy and independent life.

Longevity congratulatory money provision/ 敬老祝金の支給
The City provides a longevity congratulatory money to those residents aged 77, 88, 99 and 100 years continuously living in Misato City for one year or more as of the base date of August 31.
Subsidy for Rojin (senior) Club/ 老人クラブの補助
Subsidies will be provided to educational learning programs, sporting and social service activities conducted by each Rojin club.

This program provides health exercise classes for those aged 60 or over to maintain mental and physical health. The classes are held approximately twice a month, 18 times a year. Each session takes approximately 1 hour and 30 minutes and offered at facilities throughout Misato City.

In principle, anyone who are aged 60 or over, healthy, have work motivation and ability can enroll in the Silver Jinzai Center (an annual fee of 2,400 yen). Members work helping each other by using of their experiences, abilities and skills.

How to Pay the Long-term Care Insurance Premium/ 介護保険料の納付方法

There are two ways to pay the premium:
[Special Collection/ 特別徴収]
For those who receives pension benefit 180,000 yen or more in a year, the premium will be deducted from pension benefit. (Procedure is unnecessary.)
[General Collection/ 普通徴収]
Pay by payment slip or bank transfer other than by special collection.
* You can pay the premium at convenience stores. The amount of Category 1 insured person will be determined by the City based on the Long-term Care Insurance plan.
Category 2 insured person/ 第2号被保険者
Pay the premium as a part of your health [medical] insurance payment you enrolled. The premium amounts may differ depending on your health [medical] insurance plan.

In order to use long-term care services, you need to apply for “Certification of needed long-term care” and receive the certification that you request for long-term care services and supports. Long-term Care Insurance Division at the City Hall or Comprehensive Community Support Centers will accept your application. There are approval categories, ‘needed support level 1or 2’, ‘needed long-term care level 1-5’, and ‘Not applicable’.

If a person with necessity of long-term care or necessity of assistance wishes to use the services of long-term care (long-term care prevention), you need to create a long-term care (long-term care prevention) service usage plan. User’s co-payment and high cost long-term care service expenses/ 自己負担分と高額介護サービス費
When you use long-term care services, you need to pay 10%, 20% or 30% of co-payment of the serve charges. However, if the expenses for the service used, fees for meals, accommodation (room charges), and other expenses charged by the admitted facility exceed the user’s ceiling amount defined by the degrees of necessity of long-term care, the excess amount must be borne by you.
For those with low income, there is a system to reduce the fees for meals and accommodation of the admitted facility. If your monthly charges exceed the ceiling amount (maximum amount) which is designated based on user’s incomes, the excess amount will be paid as the high cost long-term care service expenses upon application.
*Application accepted for two years’ charges.
