This division accepts various applications, provides consultation and collects insurance fee for medical system conducted by Saitama Kouki-koreisha Organization (medical federation) at the city hall for those aged 75 or older, or those aged 65 - 74 with a certain disability certified by the organization.

In principle, anyone who are aged 60 or over, healthy, have work motivation and ability can enroll in the Silver Jinzai Center (an annual fee of 2,400 yen). Members work helping each other by using of their experiences, abilities and skills.

The Silver Jinzai Center welcomes requests from private enterprises and households for the following types of work.

How to Pay the Long-term Care Insurance Fee/ 介護保険料の納付方法

Category 1 insured person/ 第1号被保険者

There are two ways to pay the tax:

[Special Collection/ 特別徴収]

For those who receives pension benefit 180,000 yen or more in a year, the tax will be deducted from pension benefit. (Procedure is unnecessary)

[General Collection/ 普通徴収]

pay by bill or bank transfer other than by special collection

* You can pay the bill at convenience stores. The amount of Category 1 insured person will be determined by the city based on the Long-term Care Insurance plan.

Category 2 insured person/ 第2号被保険者

Pay the fee as a part of your health [medical] insurance payment you enrolled. The fees may differ depending on your health [medical] insurance plan.

In order to use long-term care services, you need to apply for “Certification of needed long-term care” and receive the certification that you request for long-term care services and supports. Long-term Care Insurance Division at the City Hall or Comprehensive Community Support Centers will accept your application. There are approval categories, ‘needed support level 1or 2’, ‘needed long-term care level 1-5’, and ‘Not applicable’.

Flow to start using ‘Long-term Nursing Care Service’/ 介護サービスを利用するには

1. Application

When you need a support and long-term care, make an application at the city’s service counter by yourself or a family member. Comprehensive Community Support Centers do the paperwork as an agent, too.

2. Home-visit survey, home doctor’s opinion document

Survey staff visits the person in care and surveys the person’s mental and physical conditions according to the nationally standardized survey items.

At the request of the city, home doctor will make a written opinion.

3. Approval examination

Based on the result of home-visit and the home doctor’s opinion document, the Approval on the Necessity of Long-term Care committee consisted of experts of health care, medical care, and welfare, judges whether or not he/she needs long-term care and support or not.

4. Approval notice

Approval categorizes will be determined as necessity of support 1or 2, necessity of long-term care 1-5, or Not approved.

The result will be notified approximately 30 days after application, depending on the questionnaire and the contents of doctor’s opinion document.

5. Services to prevent daily dysfunction of long-term nursing care, Planning, and care management

For those categorized necessity of support 1or 2, a health nurse, etc. of Comprehensive Community Support Center makes service plans to prevent becoming in a condition of being cared.

For those categorized necessity of long-term care 1 - 5, an expert of the long-term care support business makes plans for long-term care.

No co-payment will be charged for the documentation.

6. Providing long-term nursing care services

Long-term care services or services to prevent becoming in a condition of being cared will be provided based on the plan etc.

For those who wish to admit to a nursing home, please apply directly to the facility.
